Athletic Registration
Next Physical with the School Doctor will be for Fall: May 22nd and July 17th.
We need all packets filled out and returned to Athletics prior to your appointment druing School.
*Summer Physicals you need to bring your signed NJ State Physical Packet, to your appointment.

If you have created an account with a sibling, you must use that account to register a new sibling.
The program will give you three tries to put in your password, then lock you out, email Michele Grieves.
For High School Athletes please ask your son or daughter if they have changed the password.

NJ State Physical Packet must be given to the Athletic Department or School Nurse once a year.

A physical is valid for one year. You will receive an automated reminder email within 6 weeks of expiration.
Statisticians: ONLY the Health History Update Questionnaire is required.
